Mail the request form with 2 UPC’s from any DanActive or DanActive Light package by March 10, 2013 to receive a free family sticker set for your vehicle.

Print the actual form and fill it with the required information completely.
Write your name, full address (including your postal code) and telephone number
(including the area code) in block letters on an 8½” × 11” sheet of paper.
Please select up to six characters.
Stick or staple 2 original UPCs from any DanActive or DanActive Light package onto this
sheet. Only original UPCs will be accepted; all photocopies will be rejected.
