free sports physicals April 2010 – Des Moines, IA only

WHO:
ATHLETES IN MIDDLE OR HIGH SCHOOL
from Des Moines area schools needing a sports physical.

WHEN:
Do to some schedule changes, we are unable to offer this service this year. The next time we will offer free school/sports physicals on our campus will be April 2010. We apologize for any inconvenience this may cause. The Des Moines University Clinic will still offer before-school physicals but they will not be free. If cost is an issue, please visit the Free Clinics of Iowa.

WHAT:
FREE SPORTS PHYSICALS
including EKGs (and ECHOs if necessary) to screen for cardiac risks. Each student must bring a completed and signed parental consent form and physical history forms (both A & B), Parents do not need to attend the event if consent form is completed. Males are advised to wear shorts and t-shirts for the exam to make it easier to do the physical. Females are advised to wear shorts and preferably a sports bra under their shirt.
