I didnt see where it said you get an additional $10 – am I looking in the wrong place?

I think this promotion expired Dec 7th. When you put in at least $40 and choose gift card, a receipt prints out at the end with a code that you apply at checkout. At the bottom of the receipt is a rebate form that you fill out and mail in for the additional $10.
