Well this about the best I can come up with ... two ways of doing it. I have not tried these, and typically I find you often have to play with the recipe a little to get it where you want it.

To 1 cup of hot coffee (brewed or instant), add :

1 tablespoon of chocolate flavor syrup

1 tablespoon of Irish Crème flavored powered creamer

Add a little milk (2% or whole) to make it creamy.

Or to make up a big batch ...

14 ounce can sweetened condensed milk

1 1/2 c skim milk

1 T powdered cocoa

1 tsp vanilla extract (preferably real -- if imitation, use 1-1/2 t)

1 tsp rum flavoring (found in baking & spice section)

Place in a 1 qt container with a lid, mix well and store in refrigerator for up to 2 weeks (always shake before using)
