Thread: Post-Prison Quality of Life Survey-$5 McDonalds GC

For those that have spent some time in Jail, here's a survey for you . . .

As a participant in this study, I will spend approximately 30 minutes today completing six short questionnaires. I will be asked for information such as my age, gender, race, marital status, as well as questions regarding my current and past incarceration history. Additional questionnaires will ask questions about my attitudes, personality, behavior, living habits, and my thoughts and feelings.

Participation involves no future obligation. I understand that I have the option to be compensated with a five dollar ($5.00) gift card for my participation in this study, provided I agree to provide my name and address. I understand that if I provide my name and address, this information will not be connected to my survey data, and the information will be destroyed six (6) months after the gift card is sent. I understand that I will not personally benefit from my participation, and the research may benefit society through increasing our understanding of factors that increase post-incarceration quality of life.
